Happened to me as well. In my case, the menu entries for applications
installed in the system did not get generated on the fly, as
"update-menus" did not work due to a broken libstdc++ (Debian/unstable).
xfce4 (whatever part of it) did write an error message into the
~/.xsession-errors file, that's how I find out what's wrong. I think it
said "can't process element Debian". I'm not sure, and can't copy-paste
it, ase ~/.xsession-errors got overwritten in the mean time.
So, check you error-log (wherever it is), and whether your menu gets
generated (if it's generated on the fly when packages are installed).
