I think you are wrong, the first window doesn't disappear (in fact, the
window manager doesn't hide apps windows like this, it has to be a user
actions, like iconifying or switching desktop)

Anyway, the bug you describe, I guess, was in mozilla 1.2, it has been
fixed in 1.3 I think.

Typically, the specs says that a window that sets its "transient_for"
property to None or root must be considered transients for all windows
in the same group. And the "Save file as..." window is in the "Mozilla"
group, so the "What should Mozilla do with this file?" dialog shows
above (as it's a transients for the group) 

I'm on the way of implementing a smarter way of handling group
transients in xfwm4.

I don't know about gkrellm2. Sounds like some kind of race condition in
gkrellm, but I did not check the code.
