New mixer

Hey,

here's an update of the mixer. It now supports switches and options and
depends on gstreamer-plugins-base 0.10.2 instead of 0.10.14 (which is
not available in some distributions yet). It also ships its own icons. 

Things still missing:
 
  * The "Select Controls..." dialog
  * Remember the last window size
  * A better position for the lock/unlock channels button
  * Monitor for changes made by other programs and update controls
  * Panel plugin
  * MCS plugin
