keyboard map error

I have 2 installs, one debian 3.0 and one Libranet, based on debian. I 
have xfce4 installed on both, but the keyboard key output from the 
shifted number keys is incorrect. Here is the shifted top row, starting 
at the number one:

! " § $ % & / ( ) = ?

notice this is only 11 symbols. The shifted equals key does nothing on 
the first press, then outputs ´ (tick) on the second. Also, the right 
square bracket key is incorrect.

Anyone have a clue why both machines would have incorrect key mapping?
