ad SAME directory: yes i had this thought today too. a copy of the real fs
under  .thumbnails would be probably more effective. but i think you have to
count with multiple devices not just your hardrive. something like:
~/.thumnails/[UUID1]/large or small thumnails
~/.thumnails/[UUID2]/large or small thumnails
~/.thumnails/[UUID4]/large or small thumnails
~/.thumnails/[UUID8]/large or small thumnails
