Now finally I have found a test case to reproduce:

Pre-requisites:
===============
You need to have shortcuts defined for custom actions assigned to directories
(e.g. open terminal here). I have not tested if custom actions assigned to
files are affected by the same problem.

Steps to reproduce:
===================
1) Open a thunar window.
2) Look at menu File, ensure the custom actions are listed with their shortcuts
there.
3) Create a directory, e.g. with context menu->New->Folder.
4) Look at menu File, behold the custom action shortcuts are gone!

Additional information:
=======================
The shortcuts are not lost permanently. If you open another window, the
shortcuts will appear there.

Also, the same thing will happen if one deletes an existing folder. Shortcuts
gone.
