Summary: when 'focus follow mouse' is chosen, moving the mouse fast will not yield the right window focus
Description:
If I have three windows overlapping diagonally and move from one to the
third passing the middle one, and rest the mouse over the third one, I
don't get the focus the the third window. I stays stuff to the second one
instead.

xfwm4 notifies the app that it should get focus, and the app is responsible
of actually setting focs. This might cause some race conds between the
apps themselves, but that's the standard correct behaviour.

I don't think it's a bug in xfce, and I don't think it's avoidable.

Great. That explains the bahaviour. I did test it with GNOME and it had a
similar result, which confirm your idea. This is not a bug after all.
Thanks
